My Accountpage --> Table is shifting and collapses with right column

The accountpage is offset.

After ordering and reviewing with an account the order-table is offset. It collapses with the right column (see attached image).

I removed in the customer/orders.phtml the table "Shipped to" and placed it in app/design/frontend/default/jm_topaz/template/customer.

The site still generated the table and thereby it collapses with the right column.

I attach the orders.phtml (rename the file) and the screenshot with the problem.

Please post the login details of the customer where you see the order page like in the screenshot.
