The only folder in the zip file is /app. There is no ub-tool folder to install.

The ub-tool folder is automatically deployed and created after you install our migration tool successfully.
Could you please try to install again following our Readme.html? If you run across any issue then, please do let us know.

When I follow the instructions in Readme.html, it get the following error:

That seem you have installed older version of our tool in first time and you have deleted/loss the source code of our tool located at pub/ub-tool/
For that case, you should re-install a new Magento2 fresh instance with latest version and then install our tool by steps indicated in the guide in Reame.html
