File was not uploaded error

When I try to upload images to the products I get the error "File was not uploaded."
I have tried the following which hasn’t worked:
Tried using the dull uploader (no flash) same error
Tried different browsers and different computers same error
Tried using an older (1.6) prototype.js file -- same error
Renamed .htaccess in the media folder to .htaccess-old and also tried deleting it
All my permission were set according to the installation instructions, I’ve tried changing it to 755 rather than 777 -- same error.

Please assist.
