The uscoding support team say I will modified the theme.
How can I modify it ??

Please contact with the “Magento Customer Attributes” extension provided by the Using code. We do not support the third-party if you have an issue with it.
Also: Please try to check the compilation via admin enable or not. If enabled please turn off. and let install your extension.
